Improving Food Access, Improving health
We know food insecurity affects health in profound ways, and that improved food access leads to improved health outcomes. That’s why we’ve teamed up with MassHealth’s flexible services program to specifically address hunger and patient health. We work with patients referred to us due to food insecurity and medically complex health issues. Over six to nine months, we help increase their long-term food security by providing access to food through a variety of goods and services ranging from grocery delivery to kitchen supplies and more.

Referrals to our Health Care Partnerships program come from thirteen health centers that are part of the Community Care Cooperative, a group of community health centers in Central and Eastern Massachusetts, and the Boston Children’s Hospital.
We provide individualized support to these food insecure patients with medically complex health issues by addressing social factors, that play a significant role in their health.
Depending on need and geography, members may receive services to help improve their capacity to purchase and prepare healthy foods, including: gift cards to area supermarkets; transportation assistance; procurement of kitchen supplies; cooking classes; nutrition counseling; farmers market coupons; home delivered groceries; referrals for medically tailored meals; assistance connecting with federal nutrition assistance program like SNAP and WIC; and referrals for community-based food pantries.
