About Flexible Services
The Commonwealth of Massachusetts received a five-year waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members. This waiver led to the establishment of a pilot Flexible Services Program. Through the Flexible Services Program, Accountable Care Organizations may pay for health-related nutrition and housing supports as part of their strategy to address social determinants of health.
Project Bread is an approved nutrition service provider for the ACO Community Care Cooperative. In this role we will receive referrals and provide nutritional services to members from 14 health centers across Boston and Central Massachusetts. To assist us in meeting our mission, Project Bread is seeking a Program Assistant to join the health care partnerships team.
Supervisor: Director of Health Care Partnerships
Role: The Health Care Partnerships Program Assistant will play an essential role in supporting the flexible services program. Nutrition Services Coordinators will speak with clients referred to us by our health care partners and determine which goods and services are needed. The Program Assistant will then fulfill service orders to ensure that clients receive the needed items. Additionally, the Program Assistant will speak with clients upon completion of the program to assess their experience with Project Bread and the program’s impact on food security.
